All Massages are Non-Refundable; however, I will do my very best to make your appointment as excellent as possible within my capabilities and scope of practice.
All appointments scheduled will be charged at the time of appointment (with the exception that the client has signed up to become a Member or has pre-purchased a package.)

J Logan Massage Co creates and maintains paper and electronic health records describing your personal health history and all treatments. These records include symptoms, diagnoses, medical history, treatments, plans for future massage treatment, and billing and medical information.
This information is ONLY used for the following:
Planning your care
Maintaining communication with your health care provider (only if necessary)
Standing as a record for third parties (such as your insurance or my billing company) to verify services are performed with competence
J Logan Massage Co, under the HIPPA Privacy Act, must obtain written consent to disclose Private Health Information to use patients' PHI for any reasons other then routine treatment, payments, or health carer operations such as would be the following:
To Disclose personal health information about the patient to a third party (i.e. an your insurance carrier)
To Disclose records, unless disclosure of records is required by law enforcement purposes or legal mandates, oversight of the provider who created the notes, use by a coroner or medical examiner, or avoidance of a serious and imminent threat to health or safety.
All other reasons for sharing information must have written consent.
J Logan Massage does not share your information (unless necessary per above requirements) and views your information as private and personal.
All patients have the Right to Restrict any Health Information via a written document stating what they wish to restrict and, if applicable, to who.
Informed consent is the process in which clients are given all information regarding their care, including possible risks as well as benefits, treatment procedure, their rights during visit; as well as acquiring written consent to be treated.
All clients have the right (and are requested to) immediately inform the massage provider of any discomfort, pain, or lack of comfort. As well as, keep all medical information up to date in their medical file so that the massage provider may be able to offer the best informed care available.
All clients have the right to deny massage treatment at ANY time during their visit. The clients comfort if J Logan Massage Co's first priority.
The massage therapist will check in periodically to assure comfort throughout the massage session.
All clients have the right to deny ANY draping techniques at any time during the massage session.
All draping used by the massage therapist if for therapeutic and relaxing massage purposes only. This means that the massage therapist will not undrape, at any time, any region that the Clint is not comfortable with, any region not being assessed directly by massage, or any region related to any sexually suggestive part of the body.
Massage is the manipulation of soft tissue within the body such as fascia, muscle, tennis, and ligaments.
Massage often utilizes hands, forearms, elbows, massage tools, hot and cold therapies, etc for the direct manipulation of the bodies soft tissues. The intention for massage is ALWAYS to reduce, pain, discomfort, stress, tension, any ailments associated with muscular discomfort, as well as aid in well being, ease of movement, rest, sleep, and all other discomforts associated with the soft tissues of the body.
Massage should NOT be construed as a substitute for medical examination, diagnoses, or treatment of a physician, chiropractor, or other qualified medical specialist. Massage and bodywork is not a qualified modality for skeletal adjustments, diagnoses, prescriptions, treatments of physical or mental ailments by which a qualified provider is required.
Massage should not be related to ANY illicit or sexually suggestive remarks or advances as this will result in IMMEDIATE termination of the session while the client will still be liable for payment of the full scheduled treatment.
